The left shortcut menu is from an older Sugar theme.  We currently default to the Atlantic theme. The Atlantic theme has the same shortcut entries on a menu popdown. Each user can change their theme in their My Profile area.

Yes.  Create another EditView list and set the parent of this new list to the Visit Type.  You then need to create a terminology dropdown list for each of the values of the Visit Type.  For example, create a dropdown list called visit_type_lunch, and specify its values.
